The goal of our free Intervention and Assistance program is to improve our clients quality of life and to maximize their autonomy. We can help with the following services:



  • Home visit to assess needs

  • Intervention services and referrals to appropriate resources

  • Escorts to medical, social and other appointments
  • Regular phone calls to assure the continuity of the services provided

  • Admissibility assessment to the different programs of Les Centres d’Accueil Héritage 
  • Assistance to fill out subsidized housing forms

  • Help to fill government’s forms (immigration, income tax, revenues, pensions, welfare, invalidity, wheel trans, etc.)

  • Assistance to find services in French (health professionals and others)
