The goal of our free Intervention and Assistance program is to improve our clients quality of life and to maximize their autonomy. We can help with the following services:

Home visit to assess needs

Intervention services and referrals to appropriate resources

Escorts to medical, social and other appointments

Regular phone calls to assure the continuity of the services provided

Admissibility assessment to the different programs of Les Centres d’Accueil Héritage

Assistance to fill out subsidized housing forms

Help to fill government’s forms (immigration, income tax, revenues, pensions, welfare, invalidity, wheel trans, etc.)

Assistance to find services in French (health professionals and others)
