The goal of our free
Intervention and Assistance program is to improve our clients quality of
life and to maximize their autonomy. We can help with the following
services:
- Home visit to assess needs
- Intervention
services and referrals to appropriate resources
- Escorts to medical, social
and other appointments
- Regular phone calls to
assure the continuity of the services provided
- Admissibility assessment
to the different programs of Les Centres d’Accueil Héritage
- Assistance to fill out
subsidized housing forms
- Help to fill government’s
forms (immigration, income tax, revenues, pensions, welfare,
invalidity, wheel trans, etc.)
- Assistance to find
services in French (health professionals and others)
