The goal of our Supportive Housing Program is to help people who are frail, elderly maintain their autonomy and live in their own home as long as possible.

To be eligible, it is necessary to be an adult and to live at La Place Saint-Laurent and to be in loss of autonomy. The clients must need the services on a permanent basis and to need  the services at least once by 24 hours. The services are available 24 hours a day.

 

Services provided:

  • Client’s need assessment
  • Coordination of services as needed
  • Information and referral to appropriate resources
  • Coordination of social, medical or other appointments
  • Regular follow-up
  • Homemaking and Laundry
  • Meals and snacks preparation or Meals at the dining room
  • Socialisation/Exercices
  • Pratical help in activities of daily living provided by Personal Support Workers who have obtained a college diploma
  • Personal Support Workers on site at all times
  • Emergency response system, 24 hours a day

Our qualified Personal Support Workers are trained to provide
the following services:

  • Assistance with medication
  • Homemaking and laundry
  • Meals and snacks preparation
  • Social support: conversation, walks, games, exercises and escort to activities
  • Security calls or visits



There is no fee for this service.
